spec: banner stale sections so partial readers see the pivot
§0 Revisions notes that §3 and §7 Phase 2 are pre-pivot, but a reader deep-linking into either section bypasses §0. Add a section-level banner at the top of each that points back to §0 and to bundles/AGENTS.md for the current per-bundle convention. Content is preserved as a record of the original design.
